MEDICINE USE REVIEW
If you’ve been taking prescription medicines for some time, it’s likely you may have questions about whether you are getting the most from them.
The NHS Medicine Use Review Service, funded by the NHS and available at MPS Pharmacy allows you to ask our pharmacist all those questions during a private consultation, helping you to understand your condition and the medicine you’ve been prescribed.
WHO WOULD BENEFIT FROM THIS SERVICE?
If you suffer with one of the following conditions, our pharmacist may recommend the service:
Respiratory conditions such as chronic bronchitis and emphysema
Type 2 diabetes
High blood pressure
If you’ve recently been discharged from hospital and had your medication changed whilst there, you would also benefit from the service.
HOW DOES THE SERVICE WORK?
You’ll be invited to have a private consultation with the pharmacist at a mutually convenient time. As part of the consultation you’ll need to complete a consent form. Simply print out the form available to download below, fill it out and bring it with you to your appointment.
During the private consultation you’ll be able to ask our pharmacist:
